VNA’s team of experts work together to provide a number of services to patients in their homes during illness or recovery, including skilled nursing, rehabilitation, medical social workers and personal care. We also provide elder care services, such as companions and care management.
- Perform in-home assessments ensuring a proper care plan is administered, including pre-operative assessment, and to evaluate patient safety
- Collaborate with patients, their families and physicians, hospitals and pharmacies to form an appropriate plan of treatment in the home
- Educate and assist patients and families on disease management, including pain management and teaching of medication administration
